Assisted Living residents enjoy the beauty of the community, the pleasure of great friends and caring support where it’s needed. In addition to having access to 125 acres and a beautiful pond, residents are welcome to enjoy amenities such as the library, the billiards room, multiple dining venues, several living room gathering spaces, various cultural arts programs within the Auditorium and learning opportunities within our many classrooms.

In addition to assisting with activities of daily living, Collington provides daily housekeeping, laundry services, maintenance, nursing care, scheduled transportation, 24-hour security and so much more. A tailored approach is designed to enrich your’s or your loved one’s life with dedicated programming and activities, surrounded by a welcoming community of peers.
Collington is also home to the MedStar Health Center for Successful Aging Outpatient Clinic. This clinic is open to all residents and includes both inperson and telehealth appointments as well as 24-hour access to reach a physician by phone.
Aging with Choices
With an Assisted Living license offering three levels of care, Collington can provide high level care, short-term care, and long-term custodial care for residents — including those needing memory care. Residents are able to receive long-term care within one of Collington’s Assisted Living neighborhoods — Brandywine, Potomac and the Arbor. Collington also offers short-term stays to those residents with short-term care needs within these neighborhoods as well as within the Shenandoah neighborhood. Residents benefit from three meals a day, laundry and cleaning services, nursing services, outpatient rehabilitation services and wellness activities available 24/7 within Assisted Living. Hospice and end-of-life care can also be provided within all of the Assisted Living
neighborhoods.

Assisted Living at the Creighton Health Center offers the highest-quality services tailored to each resident’s specific needs. We offer four neighborhoods to ensure your loved one receives just the right balance of support and independence:
The Brandywine
Apartment-style suites including supervision of daily living activities, allowing residents to maintain their independence. Services include meal preparation, housekeeping, medication assistance, laundry and daily check-ins. Brandwine residents also enjoy accessibility to all community amenities and activities led by Independent Living residents.
The Potomac
Hands-on maximal assistance with activities of daily living, including bathing, medication assistance and dressing with 24-hour care and oversight including RN Service, bedside physician visits, and PT/OT/ST services. By customizing care for individual residents, we can help ease lifestyle transitions while respecting our residents’ individual preferences.
The Shenandoah
Our highest level of enhanced assistance, this Level 3 neighborhood is designed to care for residents with more clinically complex medical needs, including those returning from hospital stays. These private rooms include 24/7 access to a Collington RN/LPN.
The Arbor
Specially designed and structured to serve residents with Alzheimer’s disease or other dementias, memory care services ensure residents’ safety and security while helping them reach their highest potential through stimulating activities and a welcoming community of friends and neighbors. All team members have completed dementia specific training in this secure neighborhood, which is surrounded by a therapeutic garden and a spacious, scenic courtyard for venturing. 24/7 RN services, as well as PT/OT/ST and on site physician visits are also included.

Respite Care
Opportunities are available for short-term care in the Creighton Center for Assisted Living. This is a great option for those recovering from an illness or surgery as well as helps caregivers who need to travel or attend to other business by providing them a restful break. The process involves a medical screening prior to admission and is based on availability. A short-term stay is a great way to enjoy all of the amenities and services while receiving temporary care in a beautiful setting.

Assisted Living Level 1 — Light services include occasional support with two or more activities of daily living. Resident should be able to manage independently with support form a mobiity device, does not require mechanical device assistance to stand from a sitting position, and is indepently functional with medications.
Assisted Living Level 2 — Traditional services include substantial support with two or more activities of daily living. Resident requiring contact guard assistance or minimum assistance with a device for mobility, minimal mechanical device assistance to stand from a sitting position, and is provisionally functional with medication reminders.
Assisted Living Level 3 — Enhanced services include comprehensive support for any number of activities of daily living. Resident requires moderate to two-person assistance with mobility, mechanical device for transfers or to stand from sitting position, and full support for medication management.
Memory Care — The Arbor is Collington’s Memory Care neighborhood where a complete range of Dementia care programming and services are designed to meet the needs of both the residents and their families. The Arbor provides a safe and secure environment where residents are encouraged to participate in a variety of stimulating activities. Programming includes individual choices, planned and special events
designed to promote independence.
Collington also charges additional fees for medical supplies and ancillary services. These are charged at the prices in effect at the time of use. Private duty nursing costs are based on the individual contracts. Other fees may be charged, depending on the services or items provided. A list of the most common ancillary service charges is available upon request.
The Health Services Staff would like to invite you to attend Care and Service Plan Conferences to coordinate and understand the care provided to you or your relative. We will let you know in advance when these conferences are scheduled and value your participation.

How does Collington determine when someone needs additional care?
It’s easier to make the most of today, knowing you have a smart plan in place for the future. That’s the idea behind Life Care at Collington.
Resident Level of Care is determined by:
- Medical Assessment: Collington’s Health Services interdisciplinary team provides a comprehensive health assessment to help determine a resident’s level of care. Assessments also take place when there is a health care event or significant change in a resident’s condition.
- Assessment Tools: The interdisciplinary team uses a comprehensive collection of tools and measurements to assess the physical, psychosocial, and financial factors impacting a resident.
- Interdisciplinary discussion and care plan conferences: The resident and team members engage in conversations to discuss a plan of care and decide together whether that includes a change in Assisted Living level of service. Along with the resident, these conversations can include resident representatives, family members, and physician or other health care providers.
Skilled Nursing
Should a resident have a post hospitalization acute care need that cannot be provided within Assisted Living or through partnership such as home health or home care, Collington has partnered with two skilled nursing facilities within the area. Collington’s Care Navigation team will continue to be involved in a resident’s care at this facilities by being a resident advocate, participating in care plan meetings and being involved in discharge planning with the goal to have the resident return to Collington as soon as medically approved for discharge.
