Vitalize 360 is a program that combines an innovative, person-directed approach to wellness coaching with the power of information derived from a scientifically grounded assessment system. The program engages, challenges and inspires residents to live full, healthy, vibrant lives and enables Collington to demonstrate significant improvement in successful aging.
Vitalize 360 is a joint venture between two not-for-profits: Kendal and Hebrew SeniorLife, a Harvard Medical School Affiliate. Collington has collaborated with Vitalize 360 and offers this program under the direction of our full-time Vitalize 360 Coach, Kim Rivers.
Reflective, Person-Centered Coaching
Person-centered coaching is paramount to this program. When residents meet with their Vitalize 360 coach, they discuss what is most important to them, set goals and refocus on living more intentionally. Vitalize 360 supports residents as they progress through stages of “activation” toward living their “best lives.” These conversations focus on “what matters most” — not on “what’s the matter.” Goals that residents would like to achieve could be anything from: “I want to be a better listener,” or “I want to swim four laps, three times a week,” to “I want to sky-dive before I’m 80.”
Residents Create A Vitality Plan
Residents, along with their coach, create a personalized well-being road map (Vitality Plan) that guides the achievement of self-identified goals. This Vitality Plan may support existing activities or inspire new achievements. Information about the resident is gathered using two evidence-based tools — the Lifestyle Survey and the Health and Social Checkup. The Lifestyle Survey is self-administered and captures each resident’s preferences and satisfaction with current activities. The Health and Social Checkup includes items in four key areas: community and social life, cognition and mental health, getting around in everyday life, and health. Information is compiled, documented and used for tracking, benchmarking, education and quality improvement.
Commitment to Improve Healthy Aging
Collington has a strong commitment to improve the vitality of its residents. Vitalize 360 increases the quality and depth of engagement between staff and residents. We know the interests, preferences, wellness and challenges of residents, and, as a result, we can target the right programs and resources for improved well-being and successful aging.
